Complementary Cancer Care receives a variety of Grants to help fund our programs. The demographics information below will help us to provide both research and statistical information when seeking funding along with allowing us to gain insight so we can better shape our programs to directly address the personal needs of clients.

The information you provide will remain confidential.  Any names or other identifying information provided by you will be removed from any reports sent to our Grantors.

Please note – several of our classes/events allow drop-ins and you do not need to register online to attend that class/event.  However, we would appreciate it if you would register for your class/event to help us maintain an attendance count for our programs.  Thank you!

I understand that proper care shall be taken for my safety and well-being; I also understand that I am responsible for adjusting my practice to avoid injury. I assume all risk of damage or injury that may occur to me as a student by participating in the Mind-Body Classes. I release and discharge Complementary Cancer Care, Inc. and any of the Instructors from any claims, damages, demands, and actions of any nature that result from my participating in these classes.
